We only accept new passport applications; Passport renewals are done by the applicant.
We accept applications by Appointment Only from 7:30 AM until 4:30 PM Monday-Friday.
If you are not able to speak, read and write English, please bring an interpreter.
Appointment Checklist
- Completed Application Form
- Must be completed in BLACK INK
- Must have no errors (if there are errors a new application must be completed)
- Proof of Citizenship (Certified Copy of Birth Certificate, Certificate of Naturalization, etc)
- If applicable, certified document of name change (Marriage License, Court Order)
- Proof of Identification (Valid State ID, Valid Passport, etc)
- If applicable, certified document of name change (Marriage License, Court Order)
- Passport Photo (within last 6 months)
- WE DO NOT TAKE PASSPORT PHOTOS
- Minor Applicants
- 15 years or younger must have BOTH parents present with valid identification.
- 16-17 years must show parental awareness when applying.
- Check or Money order for Application Fee (Payable to US Department of State)
- Cash, Check or Card for Execution/Acceptance Fee (Payable to Cottonwood County)
